Position Title: Supportive Housing Community Support Worker (CSW)
Reports To: Clinical Supervisor
Position Summary
The Supportive Housing CSW provides on-site, client-centered support within supervised housing program, where participants typically remain for 4–6 months. This role focuses on helping participants build a strong foundation for independent living through the five CCSS functional domains: living, learning, working, social, and recreation. The CSW supports recovery, wellness, and self-sufficiency by providing skill-building, resource linkage, crisis intervention, and goal-oriented guidance, preparing participants to successfully transition to more independent settings.
The primary work location and designated reporting site for training will be assigned by the direct supervisor. All location details, scheduling information, and reporting instructions upon hire or as operational needs change.
Responsibilities

Engage with participants daily to provide recovery-focused, supportive interactions that promote independence and personal growth.

Conduct functional assessments and assist participants in developing or updating individualized treatment plans (H2015).

Provide structured skill-building in daily living (budgeting, cooking, cleaning, hygiene, and time management).

Support learning and personal development through education on communication, problem-solving, and decision-making skills.

Encourage progress toward employment or volunteer opportunities within the working domain.

Facilitate activities that promote healthy social connections and community participation.

Incorporate recreation and wellness practices into participant routines to strengthen recovery and overall well-being.

Support participants in accessing medical, behavioral health, employment, and community resources.

Monitor participant progress, document updates, and ensure functional assessments are completed every 90 days.

Provide crisis intervention, safety support, and connection to internal or external resources when needed.

Maintain accurate, timely documentation in the EHR (within 48 hours of service delivery).

Assist participant in connecting with therapists, CPSWs, and other program staff to ensure coordinated, person-centered care.

Promote recovery, resiliency, and self-advocacy among participants in a trauma-informed, strengths-based manner.

Reinforce adherence to house guidelines, curfews, and behavioral expectations while modeling respect and accountability.
Participate in supervision, team meetings, and staff training to maintain compliance with CCSS and agency standards. -
Will respond to all internal communications (email, Teams, phone, or messages) within 24 business hours, unless scheduled out of the office or on approved leave.
Qualifications

Minimum High School Diploma or GED; Minimum of 1 year of experience in behavioral health.

Knowledge of CCSS standards and the five functional domains defined by New Mexico Medicaid.

Strong communication, organization, and interpersonal skills.

Ability to engage with participants in a community setting and maintain boundaries.

Experience in crisis prevention, intervention, and de-escalation.

Commitment to professionalism, confidentiality, and ethical service delivery.
Scope of Practice
Community Support Workers provide non-clinical, recovery-focused support designed to increase independent functioning across the five life domains (living, learning, working, social, recreation). They help participants set measurable goals, build functional life skills, and connect to appropriate resources, ensuring that services align with the approved treatment plan and Medicaid standards.
Does not: diagnose, clinically assess, or provide psychotherapy; create or modify clinical treatment plans; bill outside approved CCSS codes; represent services as clinical; or provide CPSW-specific peer mentoring unless certified and scheduled in that role.
