Position Title: Intensive Outpatient Program Facilitator

Reports To: Clinical Supervisor

 

Position Summary

The IOP Facilitator provides structured group and individual counseling within the Intensive Outpatient Program (IOP) for clients with substance use and/or co-occurring mental health conditions. The facilitator delivers services following the Matrix Model, an evidence-based, trauma-informed approach that supports recovery, relapse prevention, and life skills development. The IOP Facilitator ensures documentation and service delivery align with state, federal, and Medicaid regulations.
The primary work location and designated reporting site for training will be assigned by the direct supervisor. All location details, scheduling information, and reporting instructions upon hire or as operational needs change.

 

Responsibilities

  • Facilitate Matrix Model group sessions (9 hours per week for adults / 6 hours per week for adolescents) focusing on early recovery, relapse prevention, family education, and social support.

  • Conduct individual therapy sessions and treatment reviews as part of the IOP structure.

  • Develop, implement, and update individualized treatment plans and functional assessments.

  • Document all services in the Electronic Health Record (EHR) within 48 hours, following Medicaid and agency standards.

  • Participate in interdisciplinary team meetings, case consultations, and clinical supervision.
    Support client engagement in community-based recovery support (e.g., 12-step programs, peer groups).

  • Monitor and document client attendance, participation, and compliance with IOP group rules.

  • Maintain adherence to HIPAA, 42 CFR Part 2, and agency confidentiality policies.

  • Provide crisis intervention and referrals to higher levels of care when clinically indicated.

  • Participate in ongoing training to maintain competence in the Matrix Model and other evidence-based practices.

  • Will respond to all internal communications (email, Teams, phone, or messages) within 24 business hours, unless scheduled out of the office or on approved leave.

 

Qualifications

  • Bachelor's or associate’s degree in counseling, Social Work, Psychology, or a related behavioral health field.

  • Active New Mexico licensure (LMSW, LMHC, LPCC, LCSW, LADAC, LSAA, or equivalent).

  • Minimum one (1) year of experience facilitating group or substance use disorder treatment preferred.

  • Familiarity with the Matrix Model, ASAM criteria, and Medicaid documentation standards preferred.

  • Strong group facilitation and motivational interviewing skills.

  • Ability to maintain accurate documentation and professional boundaries.

  • Will respond to all internal communications (email, Teams, phone, or messages) within 24 business hours, unless scheduled out of the office or on approved leave.

 

Scope of Practice:

IOP Facilitator (Behavioral Health Professional)
In New Mexico, IOP Facilitators provide structured, non-residential treatment services consistent with ASAM Level 2.1 care. Under the direction of a clinical supervisor, facilitators implement therapeutic and psychoeducational interventions that address substance use, co-occurring mental health symptoms, and relapse prevention. Facilitators work collaboratively with licensed clinical staff to deliver evidence-based care, maintain documentation in the EHR and client portal, and uphold the ethical and professional standards defined by the New Mexico Counseling and Therapy Practice Act (NMSA §61-9A-5) and applicable Medicaid guidelines.

 

Does not: prescribe or recommend medication; practice independently without required license/supervision; perform or bill CCSS/CPSW services; delegate clinical documentation or assessment to unlicensed staff; alter or backdate records.
