Position Title: Comprehensive Community Support Services (CCSS) Worker
Reports To: CCSS Supervisor / Program Manager

 

Position Summary

The CCSS Worker provides community-based services that promote client recovery, independence, and skill development. This role focuses on psychosocial rehabilitation, resource linkage, and coordination of care across the five functional domains defined by New Mexico Medicaid.

 

Responsibilities

• Conduct functional assessments and assist in developing service plans (H2015).

• Provide skill-building in independent and community living, employment, education, socialization, and recreation.

• Support clients with accessing medical, behavioral, and social services.

• Monitor client progress and update functional assessment every 90 days.

• Provide crisis intervention and link to appropriate services.

• Maintain accurate documentation in EHR within 48 hours.

• Collaborate with therapists, CPSWs, and community providers.

• Promote recovery, resiliency, and independence in clients.

 

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field; OR 2 years of experience in behavioral health.

• Training and knowledge of CCSS requirements and the five functional domains.

• Strong communication, time management, and advocacy skills.

• Ability to work independently and in community settings.

• Commitment to confidentiality, ethics, and professional boundaries.

 

Compensation & Benefits

• Compensation:

  • $30 per billable hour (Individual Only)

  • No Mileage reimbursement
